I understand that PAWSOME SMILE will take all necessary precautions to ensure the safety and well-being of my pet during the procedure. However, I acknowledge that there are inherent risks associated with any cosmetic teeth cleaning procedure.

Therefore, I release PAWSOME SMILE, as well as its employees and affiliates, from any claim, damage, loss, or injury that may result from external factors unrelated to the procedure itself.
